I give permission for The Portland Oregon Women's Film Festival (POWFest) and MetroEast Community Media (MetroEast) to use my child or ward's image for broadcast on their channels, as well as distribution on the Internet, public screenings and in promotional materials about POWFest/MetroEast and their services.

I understand that these images will be obtained during the child's participation in the POWGirls program. In addition, I also give permission for my child's video work to be used as programming on MetroEast channels and in promotional materials.

To retract my permission, a request to remove this form from POWFest files must be made in person or by certified mail.

Participant/Parental Disclosure and Responsibility Agreement
To ensure the success of The Portland Oregon Women’s Film Festival (POWFest) & POWGirls program, the health, welfare and safety of all youth involved will be prioritized.

All participants will be required to conduct themselves in a safe, courteous and cooperative manner while participating in workshops. This means:

• Treating all participants and instructors with respect

• Following the directions of adult instructors and volunteers

• Engaging in safe, non-violent behavior

• Careful handling and operation of equipment

I understand that the use of the MetroEast facilities includes using video cameras, microphones, audio recording and mixing equipment, lights, computers and other electronic equipment. I agree to assume responsibility for my child’s use of the facilities and equipment.

Failure to follow appropriate POWFest/MetroEast rules of conduct may result in an immediate call to participant’s parent or guardian and possible removal from the program.

Full tuition is $200 for the program--that covers 40 hours of instruction and production, equipment, a full pass to POWFest, and snacks, etc. If your family cannot afford full tuition, please complete the financial aid information below.

POWGirls strives to be accessible to girls of all financial backgrounds. Our ability to give financial aid helps the success of our organization. Applying for financial aid will neither increase nor decrease your chances of getting into the program. If you need financial assistance, please answer the following questions. Cancellation Policy
We are dependent on our enrollment numbers for our endowment and financial aid, particularly on those participants who are able to pay full price. Cancellations before March 1, 2016 may result in a refund of a portion of payment at the discretion of the POWFest board. There will be no refund of any portion of payment for program cancellation after March 1, 2016. We regret that there can be no exceptions to this policy. Thank you for your understanding.
