CGMS Technology HelpDesk Ticket
Use this form to report an issue related to your iPad or the school's technology/software. This form is for students and parents to submit a helpdesk ticket to Mr. Whitson. Once the helpdesk ticket is submitted, students will be called to Mr. Whitson's office

List your first name.
What is your current grade level?
Who is your homeroom teacher?
List your iPad Tag Number. It is on the gray sticker on the back of your iPad and begins with an "8."
Select the category that best describes the iPad issue (*CHARGERS - you must have the charging cord and/or block with you BEFORE you fill out this form. If you do not have it with you, wait until you have it! Check all that apply.)
iPad is disabled.
iPad is displaying "Connect to iTunes Store" icon.
iPad will not power on (the screen is either black or a dark blue)
iPad will not connect to internet at home.
iPad charging cord/block is damaged.
iPad screen is damaged.
iPad will not access certaing websites at home that I need for class (like Schoology, etc).
iPad will not download apps from Self Service.
Include any other information that you need to include about your iPad. This question is NOT required.
