This questionnaire aims to inform the University’s physicians about any health conditions that could pose risks during activities conducted at the MaRHE Center of the University of Milano-Bicocca.
It is therefore important, for your safety, to complete this questionnaire carefully and provide all the requested information.
Don’t worry. All health information you provide will be reviewed exclusively by the University’s medical team and will not be shared with your professors or other non-medical staff of the university, as it is protected by privacy regulations.
If necessary, in the days following the submission, the University’s physicians may contact you by phone or email to request additional information or to schedule an in-person medical examination (if you reside in Italy).
Based on the information provided, any attached medical documentation, and any medical examination conducted, the University’s physicians may offer you and your professors advice and suggestions for activities at the MaRHE Center, such as recommending the exclusion of certain high-risk activities.

Questo questionario ha l'obiettivo di informare i medici dell’Università in merito a eventuali condizioni di salute che potrebbero comportare rischi durante le attività svolte presso il MaRHE Center dell’Università degli Studi di Milano-Bicocca.
È quindi importante, per la tua sicurezza, compilare con attenzione questo questionario e fornire tutte le informazioni richieste.
Non preoccuparti. Tutte le informazioni sanitarie che fornirai saranno visionate esclusivamente dal team medico dell’Università e non saranno condivise con i tuoi docenti o altro personale non sanitario dell’ateneo, in quanto protette dalle normative sulla privacy.
Se necessario, nei giorni successivi alla compilazione, i medici dell’Università potrebbero contattarti telefonicamente o via email per richiedere ulteriori informazioni o per programmare una visita medica in presenza (se risiedi in Italia).
In base alle informazioni fornite, alla documentazione medica eventualmente allegata e alle visite mediche effettuate, i medici dell’Università potrebbero fornire a te e ai tuoi docenti consigli e suggerimenti sulle attività presso il MaRHE Center, ad esempio raccomandando l’esclusione di determinate attività ad alto rischio.
