Please use this form to notify Lovejoy Independent School District of any updates to contacts associated to students whether it be parents, guardians, emergency contacts, or authorized to pickup adults. In the Parent Portal you can view the information that is currently on file and then use this form to update the information.

You may also use the section Add Contacts to request additional contacts be added to the students. You can add the contact once and request the students to link to this contact. NOTE: IF YOU ADDED THIS CONTACT AT THE TIME YOU ENROLLED THE STUDENT YOU DO NOT NEED TO COMPLETE THIS FORM FOR THOSE CONTACTS. THEY ARE AUTOMATICALLY ADDED.
