2025 Angel Tree Form
Hello Derry Area Families!

The Angel Tree Program is back. If your family is experiencing a financial hardship and is IN NEED of additional Christmas gifts this year, then please complete this form ASAP, the form will close once we reach our total donor number. The number of students that will be included in the Angel Tree Program will depend on the number of sponsors we have this year. The students will be assigned a sponsor on a first come first serve basis. 

Please complete a separate form for EACH child in your family that is of school age (PreK-12).

Gift pick-up will occur at various community and school locations.  Pick-up information will be shared at a later time.  Angel Tree sponsors will be given the information provided in the Google Form, the first name of the Angel Tree student, and phone number of the family. Home addresses will not be shared with the Angel Tree sponsors.
