  • Scanned copy of Birth Certificate (NSO/PSA Certified)

  • Photocopy of Passport and Foreign Visa for Foreign Students/Dual Citizens.

    1. The school may assist in processing the Special Student Permit.

  • Original Final Report Card showing eligibility for the level applied for.

  • Narrative Reports, Developmental Reports, Achievement Reports, etc. (if applicable) to give the school a holistic view of your child’s needs

  • Students who are with Guardians may request a Guardianship form

  • 2 Recommendation Forms from Guidance Counselor and Principal/Class Adviser/Teacher
