2019 Ballot Request Form

This year we are implementing an electronic ballot (e-ballot) election in order to save money and trees.

Please fill out this form if:
1) You may not have given your email to membership or you are not sure if you gave it
2) You need to update your email to receive an e-ballot
3) You need a paper ballot
4) You want to make sure that your spouse or other household member gets a second ballot ($50 minimum donation).

Members who need a paper ballot can also have the option of leaving a voicemail at 510-854-9663 with their name, number and address. 

Please fill out 1 ballot per member.

Ballot will go out August 15th, 2019 and must be received by October 15th, 2019.
