Affiliate of the Year Nomination Form
PURPOSE OF THE AWARD: This special award is given to an active affiliate member of the Park City Board of
REALTORS® who is recognized by the membership for a commitment to excellent service in his or her
respective field. Significant contributions to the real estate community will be considered when determining
the recipient. This award will be given at the discretion of the Special Awards Committee and the Board of
Directors. This award is not necessarily given annually.

The following criteria will be considered:
A. Nominee must have been a member of the Board for at least twelve months as of December 31st in the
year for which they are nominated.
B. Nominee must have had "active" participation in the Park City Board of REALTORS®, which may include
regular participation at Board events, sponsorship of Board events, and service on committees.
C. Nominee must be deemed to enhance the quality of the real estate transaction experience, adhere to
the Board’s same standards of excellence, and provide essential local market expertise.
All members of the Park City Board, including affiliates, are encouraged to submit nominations for the
Affiliate of the Year award. Self-nominations are accepted.
