I hereby warrant that I or my child, here after referred to as The Competitor, is in good physical condition and have no disability or impairment preventing them from safe participation. I acknowledge that competition will involve strenuous activity and physical contact and may be hazardous and could result in injury. I acknowledge that The Competitor is participating at their own risk and waive any liability, and relinquish all rights that they have now or may have in the future, against Fairbanks Karate and Master Cathy Fairbanks. In addition, I agree to indemnify and hold Fairbanks Karate and Cathy Fairbanks harmless from and against any and all claims, demands, fines, suits, actions, orders or damages of any kind which may result from The Competitor participation. I also release Fairbanks Karate and Master Cathy Fairbanks from any and all claims which I may hereafter have related to participation in the tournament.
