Application for Reviewer Role for the CodeGov Project
If you are a developer in the ICP ecosystem, then we invite you to apply to become a reviewer for the CodeGov team.  This form will help us determine if you have the skills we need and will make sure you agree to the terms and conditions for being a reviewer on our team.  If we have a need for new reviewers and your background looks like a good fit, then we will invite you to participate in a few reviews (paid) for a trial period.  This will give you an opportunity to decide if you want to make a commitment to performing these proposal reviews routinely and reliably and it will give the CodeGov team an opportunity to verify that you are a good fit before offering an invitation to join the team.

Voting Neuron ID
What neuron ID do you plan to use for voting on IC-OS Version Election or System Canister Management proposals?  It is recommended that you choose a neuron that is not linked to your personal investments.  This neuron only needs to meet the minimum requirements to vote in the NNS.

Reviewer Agreement
All reviewers for the CodeGov project must answer the next four questions below and must be willing to complete Form W-9 (US Citizens) or Form W-8BEN (not a US citizen).  This form can be emailed to codegov.org@gmail.com.  Do not complete this form until requested.  It is provided here so you know what personal information you will be required to provide.
