Thank you for taking the STS Qualifier test self-assessment survey. By clicking submit, your score will be calculated and explanations to each answer will be immediately provided for your review.

If you selected the option to provide additional information, you will get an email summary along with a copy of your answers. Someone will then follow up with you shortly to answer any questions you may have.

There are no right or wrong answers to these questions! However, if you answered yes to any of the first three questions, you were redirected to this page and are currently disqualified from being eligible to acquire the STS Program at this time. See the survey results for the reason why.

Yes answers in green and labeled as "correct" add 10 points to your score which implies an increased need for the STS Program. A No answer in red did not increase your score. Your total score will be shown at the top right corner of the results page after you click submit and view your results.

If your score was:

0-30 Sorry, but you don't qualify for the STS at this time.
30-50 You likely don't have an urgent need for the STS at this time.
50-70 You do have a urgent need for the protective benefits of the STS Program.
70-100 Strongly consider starting an STS Program right away. You ARE at risk!
