After-School Activity Sign Up  (Magic the Gathering)
Please feel out the form in its entirety. This activity will run from 3:00-6:00 PM on Fridays and will be held in the EC community room. Once your child(ren) are signed up, more information will be available. The start date will be Friday, 10/27.
*This club is only for 5th-8th graders

Safety is a top priority to Bright Water; therefore no child enrolled will be released from the activity without a parent/guardian signature. In case the parent or guardian cannot be reached, please provide a list of two individuals along with their phone numbers who are authorized to sign on their behalf.

Students are expected to be picked up from the after-school program and must be signed out by a parent/guardian or someone designated by the parent/guardian.  Students must be picked up promptly at the end of the after-school program.  If a student has not been picked up by the end of the program, after-school staff will try to contact the parent/guardian and/or those individuals designated as emergency contacts. Two (2) late pick-ups total will be cause for dismissal from the after-school program. Please initial below to confirm you've read and agree to the terms above. *
