Event Notification Form
Businesses, employers, and sole proprietors no longer need to complete this form for regular business  operations.

Event organizers are ASKED, but not required, to complete this form.  

Grand County Public Health (GCPH) is no longer writing approval letters for events.  By completing this form, you are simply notifying GCPH of an event you are planning.  This will assist GCPH in more quickly and efficiently containing outbreaks that may occur at events throughout the county.

Mask Wearing in Grand County:
Grand County shall align with Colorado’s current Statewide Mask Mandate and/or Guidance found at https://covid19.colorado.gov/mask-guidance.  

GCPH continues to strongly recommend masking of individuals 3 years and older when in indoor public places; and/or when 2 or more households of non-vaccinated individuals are in close proximity and 6’ distancing cannot be maintained.  

Individual businesses, organizations, and entities in Grand County have the authority to require patrons entering their facility to take COVID protective measures (such as mask wearing and social distancing) that are more strict, but not less strict, than those required by the State of Colorado, Grand County, and any authorizing municipal jurisdiction.

To assist with potential outbreak and disease exposure notifications, Grand County Public Health (GCPH) has created a generic ‘Grand County Event Contact Information’ Google Form and QR code for events in Grand County.  We STRONGLY SUGGEST you hang this QR Code Poster at your event and request that one member of each party (staff and patrons) complete it upon entry.  Information goes to Grand County Public Health and is ONLY used to assist in outbreak investigation and notification should a potential COVID-19 disease exposure be identified for this event.
