Please list which choir you were a member of, choir activities you participated in (All-State Audition Process, Solo and Ensemble, choir officer positions, choir awards, etc., voluntary performances such as Wassail, etc.), as well as other extracurricular activities, community service, honors and awards, employment, etc. for each year - freshman, sophomore and junior. *
Please email your current transcript to  StratfordChoirScholarships@gmail.com.  Unofficial transcripts are acceptable, including those printed from Skyward.  Your application will not be considered complete until the transcript is received.  Please note the following:

a)  If you are under 18, please have your parent or guardian sign your transcript.  If you are 18 or over, please sign it yourself.  This signature gives the Scholarship Committee permission to review the transcript.
b)  Black out your social security number if it is visible.
