If you would like your dancer to be able to self-dismiss from our summer program without a parent or guardian meeting them at the DDF studios for pick-up, you must enter the information below. Without this form on file, your dancer will not be allowed to leave DDF premises without a parent or caregiver pick up. By checking the box below, you hereby agree that your child may so self-dismiss from all their DDF classes/program for the Summer of 2023. In addition, by agreeing to the foregoing, you also understand and agree not to hold DDF, and that DDF cannot and shall not be held, liable or responsible for any risk to the health, safety, or physical wellbeing of any student / dancer / participant who self-dismissed, and you hereby assume all risks associated with such self-dismissal on behalf of the student / dancer / participant for whom you are responsible and release DDF from any and all liability, damages, costs, or expenses with respect to same, and covenant not to sue DDF with respect to or relating to same.  By agreeing to allow your child to self-dismiss as contemplated herein, you further agree to and hereby do indemnify and hold DDF harmless from any and all liability, damages, costs, or expenses (including reasonable attorneys’ fees and costs) incurred by DDF as a result of your violation of the foregoing.
