Thank you in advance for choosing to partake in a voluntary 12-section survey that will take less than 10 minutes to complete. The first page of this survey will be informed consent. By choosing “next”, the subject has agreed to participate and is aware of the minimal risk of participating. This survey is anonymous. All data will be collected and deleted once analyzed. 

As a participant in this survey, your knowledge regarding perceived UV-exposure and skin cancer risk will be assessed. Additional demographic information will help further analyze the data. Personal risk stratification measures will also be contributory, as well as personal or family history of skin cancer. 

At any time during the survey, participants have the right to exit out of the survey entirely and choose not to continue as a participant. 

Once the survey is finished to completion and you have answered all of the questions, you will submit your final answers and exit out of the window.
