King County 4-H Horse Program Enrollment Packet Step #2
Signing up for the King County Horse 4-H program is a simple 2 step process. Both links are on the home page of the website.
You will also need to fill out (STEP #1) the King County Enrollment form. That form requires a log in and is for parents and members (member fee is $30, parents are free)
AND
You ALSO need to fill out this form (STEP#2) ONLY for the 4-H member that is joining the Horse Program.

 This form includes the King County 4-H Horse Program Code of Conduct, Fair and State Requirements and Medical Release Forms. You will receive an email summary of your responses for your records if you entered your email correctly. 
Completion of this form is required to complete enrollment into the King County 4-H Horse Program. 
You must also enroll in King County 4-H, this is button #1 on the website home page. 
One complete form must be completed for each 4-H member, if you have more than 1 member in your household you will need to fill it out once for each member. 

Each member must complete a Horse Certificate for the project horse they will be riding and turn it in to the Permanent Numbers Chair by April 1st. 
You can find the Horse Certificate form on the website in the bubble menu below where you clicked on this link:  
  • Horse Certificate on file at the time of competition for Juniors, Intermediates and Seniors to participate in the Medals program or by April 1st if competition is after April 1.  
  • Horse Certificate must be on file by April 1st for Intermediates and Seniors to be eligible for state teams
  • Each member gets one number for each horse and rider combination from the Permanent Number Chair, members print out their own numbers (ariel font size 312).  
  • Nothing earned or won will count if you're wearing the wrong number.
  • Be sure each member understands and has access to the rule books: King County Horse 4-H Rule Book, PNW Contest Guide and the King County 4-H Medals Book are under the Publications tab on the website.  
  • The deadline for fair entry forms to be turned in to the fair superintendent is announced each year and no late entries are accepted  
  • This form will be signed by member and parent and turned in with enrollment paperwork for the member to be officially enrolled in King County Horse 4-H Program
