Remote Meal Request Form - Yellow Status Alternating Days
We know a remote learning environment may cause many hardships on our students and families. School meals can provide needed nutrition for students.

We encourage ALL families to take advantage of this service. Students do NOT need to qualify for free/reduced lunch for this service; however school lunch prices may apply to reduced or regular lunch price students. For those required to pay reduced or regular lunch prices, please keep your child's EZPay Lunch Account updated with funds. Funds will be deducted from your account if requesting a lunch. The ODE is applying for a waiver to provide meals at no costs to all students, but the waiver has not been issued by the USDA at this time.

THIS FORM SHOULD ONLY BE COMPLETED IF YOU ARE REQUESTING A MEAL FOR YOUR CHILD ON THEIR SCHEDULED REMOTE DAYS (YELLOW STATUS LEVELS). MEALS WILL BE AVAILABLE FOR YOUR CHILD TO PICK UP UPON LEAVING SCHOOL ON THEIR IN-PERSON DAYS FOR THEIR REMOTE LEARNING DAYS.
