As Parent/Guardian, you hereby acknowledge that you have received, read, and understood ASPIRE Academy of Fine Arts Summer Camp policies and guidelines provided by the organizers.       

As Parent/Guardian, you understand that the ASPIRE Summer Camp aims to provide a safe, inclusive, and enriching environment for all participants and agree to cooperate with the organizers and staff to ensure the success of this goal.

As Parent/Guardian, you understand that the camp will include activities such as workshops, performances, rehearsals, and outings.  

As Parent/Guardian, you also acknowledge that participation in these activities may involve certain risks and hazards, and you release the organizers, staff, and volunteers from any liability for injuries or accidents that may occur during the program, except those caused by their gross negligence or willful misconduct.

As Parent/Guardian, you understand that the organizers and staff will take all reasonable precautions to ensure the safety and well-being of participants, including following appropriate health and safety protocols in light of any prevailing circumstances such as pandemics or other emergencies.

As Parent/Guardian, you agree to promptly inform the organizers of any changes to the contact information provided above and to provide any additional information or documentation requested for the participant's enrollment in the program.
