DPS K-8 & Special Education Transportation Needs Survey
With the potential return to the classroom on Monday, February 22, 2021, DPS Transportation and Fleet Services must plan bus routes to accommodate all K-8 and special education students who will need transportation for the remainder of the 2020-2021 school year (February 22-June 29).

Please complete the following form for EACH K-8 or special education DPS child in your household (you can submit the form multiple times so there is a separate submission for each child).

Please note that all COVID-19 protocols will be followed on DPS buses. Students and staff must wear face masks and must maintain a safe distance from others. Buses will be loaded back to front and students will have assigned seats to ensure students do not cross one another's path and that a safe distance from others is maintained at all times.
