If you are awarding a prize, please note the winnings and the value.  (Note: the cash prize or value of the prize must not be greater than $2,000 AND if it is over $599 the winner is subject to paying taxes)
Please explain/describe what specifically you are fundraising for and please include the expected costs for said items.  Example: Uniforms, team meals, banquet etc. *
Will the funds you are raising be used for an expense in the 25/26 academic year?  *
If no, please explain.  
What are the expenses associated with this fundraiser, for example the printing of tickets, or purchasing the prize?  (depending on the fundraiser, you may be required to fill out a budget form for the business office) *
Please provide an estimate of what you expect to make in profit?   (After the fundraiser please provide detail on all revenues and expenses from the fundraiser. See sample Fundraiser Budget) *
Once approved, the OLSH Communications Office will promote the fundraiser in the Grapevine and on social media.  Would you like a flyer/graphic to be created for your promotion of the fundraiser?  
Will you be requesting start up cash from the business office? If start up cash is required, please fill out this form one and return to the business office no less than one week prior to event. Put CASH in "check payable to".
