Please use this form to notify the school of any changes to your child’s regular dismissal routine. This includes early pick-ups, changes in transportation, or going home with another student.

Important Instructions:

• Do not complete this form before 5:00 PM on the evening prior to the day the dismissal change will take place.
• If your child is going home a different way than usual, this form must be submitted by 2:00 PM on the day of the change.
• If it is after 2:00 PM, you must call the school office directly to ensure the change is communicated in time.
• If your child is going home with another student, both families must complete this form for the change to be approved.
• Please complete a separate form for each child if changes apply to multiple students.
