Club Card, Lift Ticket and Rental Form 
    Welcome to the Country Oaks Ski and Snowboard Club. You do not have to attend school at Country Oaks to join, all are welcome. Our Club night is Thursday. Please have this form completed and submitted prior to noon on Wednesday in order to ski Thursday. I accept payment via Venmo @Melissa-Kurth-5 or Zelle at 269-501-7268. I must receive your payment when you order, or I will not order your tickets. You will know if I receive your form and payment when I like your venmo. 
    Club Cards are for sale now! You may make arrangements for a porch pick up or can pick up the cards at our first meeting on Thursday, January 8, 2026.
    • Clubs begin in the week of January 6th, 2026, and meet weekly through the end of February 20th, 2026.
    • Club card rates are good Monday – Friday, Non-Holidays, from 3:00p.m. until close.
    • There are no ski club lessons on Mondays, Weekends or Holidays.
    • Parent/student cards cannot be used with any other discounts. For example, if you bring
    your 5-year-old to the window you can’t use your membership card. Children under 5
    years old can receive a free ticket with a purchase of regular full price ticket at window.
    Child must be present.
    • Use your student club card at Mt. Holly or Pine Knob. (rules & pricing vary by resort).

    Holiday Rates Dec 22, 2025 – Jan 2, 2026, Martin Luther King Jr. & Presidents Day
    weekend. ****NO student tickets or student lessons are offered during holiday times.
    During the holiday break the student can show their club membership card at the ticket window
    and receive $10 off their lift ticket. While student lessons are not available during this time other
    lessons (private, group, etc.) are available through our ski school. Physical membership cards
    must be present to receive the discount, no exception.

    Student Club Card - One student card for Ski & Snowboard with 2 lesson tickets
    Membership cards are purchased from your school club sponsor only.  

    Student members must be at least     7 years old – 12th grades. Each student must purchase a membership card to belong to your club and receive the discount. This is a yearly membership card. Card must be filled out completely with cardholder/club information to be valid. Child loses a card, they can repurchase for $30 with sponsor.
    On club nights students purchase tickets / rentals from their sponsor. Students will be sent
    away if they try to purchase on their club night at window. MUST GO THROUGH SPONSOR.

    On NON-club nights students show their card at the ticket window to purchase tickets / rentals.
    Valid after 3pm, no weekends or Holidays.

    Additional lesson tickets can be purchased with sponsor or at the instructor’s desk.
    Patches: Our lesson program is a progressive patch grading system. Patch Program STRICTLY Enforce.
    Lessons are about 50 minutes long and start at 6pm.

    • Experienced students may begin at the level that matches their ability. They do not need
    to start with the beginner lesson. 1st week is evaluations. Evaluations help sort students
    to the correct lesson. You may arrive 20 minutes early the first week if you would like to be evaluated prior to the start of the lesson at 6pm. Look for a group of instructors outside the rental shop and ask for an evaluation.
    • Patches are only given after completion of lesson.
    • It usually takes several attempts to pass each level of patch lessons.
    • Previously patches are good for current and future seasons.
    • Highest patch earned must be displayed. Example: (sewn on, key chain, zip tie or arm
    band).

    It is the responsibility of the students to make sure they are on the appropriate hill and riding the
    correct lift. Students must have the appropriate patch before attempting to ride the lifts. If not,
    students may be turned away from lifts and will need to walk back to the appropriate terrain.

    Parent Membership Card
    This card allows parents/guardians the opportunity to ski/snowboard with the student member at
    a reduced rate on club nights only. Ski club night is the only time this discount is available to the
    parents. Parents must purchase a club membership card from their school’s club sponsor.
    Tickets & rentals are purchased at the ticket window with physical card present.
    On club nights, tickets and rentals are $30 each.
    On non-club nights the card offers 10/ $5 off savings. Card must be filled out completely with
    cardholder/club information to be valid. Physical card must be present to make purchase,
    no exceptions.
