College Park High School PTSA       Scholarship Application 2019
Application Deadline: Friday, April 19th, 2019 at 3PM

Eligibility:
1. The scholarship is open to all College Park High School graduating seniors with a minimum cumulative unweighted grade point average of 2.5 who plan to continue their education at a two or four-year college or vocational school
2. Applicant must be a PTSA member. Membership dues are $10.00. You may become a member online at https://cphs-mdusd-ca.schoolloop.com/cms/page_view?d=x&piid=&vpid=1340029635714
Select "PTSA Membership" on the left and complete the information under "Student".

Award Amount:
There will be five $500 scholarships awarded

Application Procedure:
1. Complete this application on Google Form. You must complete it in one sitting as Google form cannot save your progress.
2. Ask a College Park faculty or staff member to complete a reference form linked here (https://docs.google.com/forms/d/e/1FAIpQLSeYqAxNDvYXgObfTP61p3IHzojCY71oP6y8guikApWVC1T3gQ/viewform?usp=sf_link). The faculty member prints the form, and then they must give it to you in a sealed envelope with the outside flap signed. You are responsible for delivering this reference to the College and Career Center by the deadline.
3. Print out an unofficial high school transcript and turn it in at the College and Career Center.
4. ****Print and sign application signature page when you turn in your application in the College and Career Center.

6. Double check that you have submitted the application digitally by Friday, April 19th at 3:00 PM and that you have handed in a paper copy of your transcript and signed the signature page at the College and Career Center. Verify if your teacher submitted your reference digitally or in paper form.

College Park High School PTSA Scholarship 2019
The goal of the College Park PTSA is to provide opportunities for all students to reach their maximum potential. We work with teachers, parents, community members, and students to ensure the needs of students are being met. To this end, we are proud to offer scholarships to deserving, graduating seniors who demonstrate both personal success and the ability and desire to share their talents with others to make a difference in their community.

Scholarships are awarded to students who embody these ideals by working within their own groups of family, friends, and community to make a difference to others. Students might achieve this by being role models to others by gaining and holding employment, by becoming involved in school activities, or by joining outside organizations whose mission is to benefit our community or society as a whole.

Please note that although a student is required to be a member of the PTSA in order to receive a PTSA scholarship, students are not required to have attended PTSA meetings or work directly with the College Park PTSA. We realize that many organizations support our ideals and encourage all students who meet the criteria to apply for the PTSA scholarship.
