Sign Up - UHMIS: DVSP Coordinated Entry
This form is meant for management to fill out for onboarding new agency staff at Domestic Violence Service Providers (DVSPs) that will need access to the Utah Homeless Management Information System (UHMIS).

The organization should be an active UHMIS organization before signing staff up for this training. If your organization is not a UHMIS participating organization, please contact the UHMIS Lead Agency (

If you have a new staff member that needs access to UHMIS, then please fill out this form. Staff will receive training and instructions sent to their email within three business days.

This training is an introductory course that is geared to brand new users at DVSPs. The training introduces key concepts and data elements that should be entered by DVSPs for UHMIS.
