Registration Instructions | 报名说明

This form may be edited after submission. Please do not submit multiple entries for the same student.
If you are registering more than one student, please submit a separate registration form for each child.
The “Email” field on the first page is used to collect registration information. The same email address may be used to submit forms for multiple students.
Please note that editing a submitted response will overwrite the original entry. To avoid accidentally replacing another student’s information, do not use the “Edit response” function to submit a different student’s registration. If you are unsure, you may use different email addresses for different students.
