Certification of Attendance Request (DMV-301 Form)
Any request before 06/22/20 or after 07/21/20 must be made to the student's school of enrollment. Only requests for Clark County School District students will be processed.
County School District (CCSD) personnel will utilize Infinite Campus to verify NRS 392.122 and CCSD Regulation 5113 attendance requirements. To ensure confidentiality and privacy are maintained, a completed form will ONLY be sent to the parent/guardian email address listed in Infinite Campus and the student's CCSD Gmail account. Only a request from the student or his/her parent or legal guardian (as identified in Infinite Campus) will be processed. Any request to amend a child's educational record must be initiated at the school level.
