MEMBERSHIP AGREEMENT/CANCELLATION
A membership term is defined by the selected option under membership term. Either autopay (month to month). Selecting autopay means that your membership will be automatically renewed every month unless cancellation is provided by any participant listed above one week (7) days prior to your membership renewal period via email to thedojo@dojosource.fit.
MEMBERSHIP TEMPORARY SUSPENSIONS
Suspension of a membership will postpone the termination date of your term by the same length as your suspension. Memberships suspended more than twice per year will incur a $25 restart fee. Requests must be made at least 7 days prior to your billing date via email to thedojo@dojosource.fit. Please make sure you receive a reply from us acknowledging that we received your request.
ACCOUNT PAYMENTS
You (signee) understand that it is your responsibility to make sure your payment is received on-time and current payment information is updated in a timely manner. Should your credit card, bank account, or other form of payment change, you will notify Dojo Source LLC at least 7-days prior to the next billing cycle. Payments not received on time will incur a $25 late fee after the third business day. Returned checks incur a fee of $50. Dojo Source LLC is not responsible for sending a reminder of any form of payment being due. Your credit card or bank account statement will act as your receipt of payment. For members that pay by check, your deposited check will act as your receipt. Emailed receipts can be given upon request.
RATE CHANGES
Membership Rates are subject to change. Rate changes will be communicated via the www.dojosource.fit website, Dojo Source App, and by email from thedojo@dojosource.fit at least 45 days prior to implementation. The rate change becomes effective upon the member renewal date (next payment period) following the 45-day notification.
MEMBER’S RIGHT TO CANCEL
If you wish to cancel this agreement, you may do so by emailing your request to thedojo@dojosource.fit within 7-business days of the first class after signing. The notice must say that you no longer wish to be bound by this agreement, and that you would like to cancel your membership. This agreement may also be cancelled if Dojo Source LLC ceases operation.
REFUNDS & STORE CREDIT
The first month is Non-Refundable and Non-Transferrable. After the first 7-business day period after your first class, any tuition payments are Non-Refundable, but may be transferred to another student or used on any Dojo Source LLC goods or services, including Uniforms, belts, equipment, camps, private lessons, tests, Ninja Nights, Birthday Parties, etc. That amount expires 60 days after the payment date.
CLASS SCHEDULE
A regular class schedule will be posted at www.dojosource.fit. Dojo Source LLC reserves the right to make temporary and/or permanent modifications to the schedule at any time, for any reason.
PRIVATE & SEMI-PRIVATE LESSONS
The standard membership now buys 30 minute Semi-Private Lessons, this means between 1 and 4  additional people will be in your lesson. Private lessons need to be scheduled beforehand and will be the same time each week. If you need to reschedule a lesson we require 48 hours notice via email (thedojo@dojosource.fit) or text (720/ 819-7572), if you don’t show up for the class or don’t give 48 hours notice you will give up this time and not receive a refund or reschedule. All make ups must be used within 60 days of the cancellation.
FREEZING & PRORATING
If you are traveling or wish to put a temporary hold on your membership you must email the dojo at thedojo@dojosource.fit 7 days prior to your renewal. A freeze will stop all payments and you will be removed from the schedule for the duration of the freeze. You will still have access to the app for 30 days after freezing. To prorate you must be gone for more than 1 week in a payment cycle and less than 3 weeks. You will be charged only for the classes you attend and prorating will incur a 10% extra charge (of your normal monthly fee) to keep your spot on the schedule while you are gone
ADDITIONAL COSTS
Uniforms, belts, equipment, camps, private lessons, testing, etc. are all additional costs and are not included in with tuition. A uniform (Gi), belt, and patch are the only required equipment for Martial Arts classes. Fitness classes require bag gloves. Gear may be purchased through Dojo Source LLC or any other supplier. Please ask your instructor for gear requirement guidance.
