Faith Formation Religious Education Registration 2024-2025
Greetings!! Thank you for choosing St. Gianna Parish Faith Formation for your family. We are excited you are here and looking forward to the opportunity to grow in faith together. We thank you for the chance to accompany your family in faith this upcoming school year. **We are looking for more catechists to help journey with our students in the classroom this year! Please prayerfully consider joining us on Monday evenings. We truly value all our volunteers and could not do this without you. We are also seeking office helpers this year if there are any high school teens who need to fulfill service hours.**

Please note, the Faith Formation balance must be paid in full from the previous year to register for the 2024-2025 school year. Registration will not be accepted with an outstanding balance. The tuition increase you see reflects a $25 increase for tuition and eliminating the separate fundraising fee. 

Faith Formation classes will meet on Mondays from 6:00pm-7:30pm. There are approximately 24 sessions for the 2024-2025 school year. The schedule of dates will be sent home and posted online once finalized and approved.

Tuition $275 (one student) $325 (two students) $350 (three or more students)
Catholic Children's Bible from St. Mary's Press $26 [One per family, for new families only].
$100 non-parishioner fee (must provide envelope number and support parish to be exempt)  
Late Fee $50 beginning August 1, 2024

Class sizes will be limited based on adult volunteers for each grade level. All adult volunteers must be in compliance with Protecting God's Children paperwork and course. This includes Mandated Reporter training as well. Students will be placed on a waitlist if class is full.

Attendance Policy 2024-2025: If your child(ren) will be absent from Faith Formation Religious Education Class please contact the office BY EMAIL for the records. Students must be present for at least one hour of class or receive an absent mark. Students who are out of class repeatedly will be required to provide a doctor's note. All students will be responsible for completing missed work so as not to adversely affect their Faith Formation. As the parent/guardian and primary catechist, I agree to make Faith Formation Religious Education Classes on Monday evenings a priority to ensure my child(ren) does/do not miss class excessively. *
I understand that joint custody will be assumed by the Faith Formation team and both parents will be able to pick up their child(ren).  If there is a sole custody situation or restrictions as mandated by the court, a copy of the legal document must be provided to us or joint custody will be assumed.

It is my child's first year for Faith Formation Classes/my child(ren) are NEW to the program and I will submit a copy of their baptismal certificate by October 1st.  *
I understand if my child is celebrating a Sacrament this year [Communion or Confirmation], I must submit a copy of their Catholic Baptismal Certificate to the Faith Formation office. This includes children baptized at Queen of Martyrs or St. Bernadette Church.   *
Book Fee: $50 per child due to complete registration. Your child is not guaranteed a spot in class until this form is completed and $50 non-refundable book fee is paid. Your child may be placed on a wait list depending on catechists. *
Bible Requirement and Fee: Each FAMILY must have a Bible for reading and discussing Scripture stories that will be shared each month. For ease of use, language, and comprehension level for children ages 5 to 14, we will use the “Catholic Children’s Bible” from St. Mary’s Press. You can order this Bible from us for $26. It should be paid upon registration. You are also welcome to order/purchase this Bible yourself from various online stores/vendors. Each family should have their Bible by October 1, 2024. *
Financial Agreement: Tuition Payments [11/18/24 and 3/10/25]. If you need to make smaller monthly payments, please contact the Faith Formation Office via email at tina@mystgianna.org.  *
Late Fee $50 applicable after August 1st *
I understand my registration will not be accepted if there is a remaining balance from 2023-2024 school year. I also understand registration is not completed until the $50 payment is made online or to the Faith Formation office. Payments can also be dropped off at the Rectory [10233 S. Central Park Ave.] *
I can be a part of the Faith Formation team and walk with students in faith this year in a classroom. I understand the commitment includes 1.5 hours of class each Sunday and some prep time for class. Classes begin September 9th and continue through April 28th *
