Companion Animal Disease Risk Management

There are occasional outbreaks of avian diseases in commercial and wild flocks, and occasional concern regarding diseases in mammals. To help us develop protocols to protect our wildlife patients and animal ambassadors, and your companion/pet/agricultural animals, we request you keep us updated on what types/species of companion/pet/agricultural animals (birds, mammals, reptiles, and amphibians) you have at home. This survey needs to be filled out when you first start at the Lindsay, and whenever you have a change in the types of animals you have at home.
