RESPONSIBILITY AND REGISTRATION FOR 2023/2024

REQUIREMENT - 1 of 3
Athletes must be registered on the following athletic contract and information form.  They are unable to participate in any practice session or contest until registration has been completed. It is only necessary to register your athlete ONCE per school year. If you have more than one child participating in athletics you are required to register each child separately.

REQUIREMENT -  2 of 3
Provide valid sports physical and health appraisal dated on or after April 15, 2023 with physician's stamp and signature to the coach at their first practice.

REQUIREMENT - 3 of 3
The Clarkston School District has instituted a pay-to-participate fee for athletics. The fee for the 2023/2024 school year for grades 6th through 8th is $200.  

Your athlete's pay-to-participate fee can be submitted at the end of this registration form through EFunds. Please print a copy of your receipt (include athletes name) and turn into coach at first practice for payment verification. This fee may also be paid with a check/money order made payable to Clarkston Community Schools and given directly to the coach.

I understand that the fee I am paying does not guarantee playing time or control over any conditions of the team, and is not refundable unless an injury takes place which disallows participation.  A doctor’s authorization letter must be submitted with a request for refund.  If the injury takes place after the midpoint of the scheduled season, no refund will be granted.  I also understand that paying a fee does not alter Clarkston Board of Education Student Policies, Michigan High School Athletic Association regulations, the District Athletic Code, and individual team rules.
