Lockyer's Middle School - Summer Term 2024 -  Club Registration/Consent Form
This form should be completed by all parents/guardians prior to their child taking part in extra curricular clubs in the SummerTerm 2024.

PLEASE NOTE:  

All after school clubs run between 3:15 pm and 4:15 pm.

All clubs will  begin from Monday 22nd April 2024 to Friday  19th July 2024.

Extra-curricular clubs will not run on Wednesday 19th June.   

English and Maths interventions after school will continue as usual up to SATs week (SATs week is the beginning Monday 13th May)

IMPORTANT: If the number of pupils requesting a place at any club exceed the limit for a given club names will be drawn to allocate a place.
