32nd District HSA Nomination Form - 2019
Recognize, Thank and Honor Our PTA Heroes
Each year, the Thirty-Second District PTA presents Honorary Service Awards to those who support families and children throughout Contra Costa County and would like your assistance in identifying outstanding nominees. This year's awards will be presented at the Founders Day Dinner on February 28, 2019.

Please nominate individuals or organizations, who are volunteers or professionals, who have made a significant contribution to PTAs in Contra Costa County. Think about who may have made a significant contribution or accomplishment in a unit, council or school that supports and serves the goals of PTA; let us know who these people are and what they've done to earn your admiration. While the Thirty-Second District PTA may be aware of individuals whose contributions are public and well-known, please also consider the many quiet contributors who "make it happen" for the benefit of the children and families in our County. All funds collected with this program are used exclusively to support the California State PTA scholarship and grant programs.

To nominate an individual or group, please complete this form and click the submit button at the end of the form by January 21, 2019. More information about service awards is on the CAPTA website, CAPTA.org. Send your questions, to shirley@paque.net.
