  • A new PWPSD student
  • A current PWPSD student who has moved locations, changed schools, or need to update contact information
Do not use this form, and contact the Transportation department directly, if your child:
  • Is a student who requires specialized supports and services
  • Is a Pre-Kindergarten rider. Pre-Kindergarten riders need to be authorized by the school. Please contact your school to discuss.
  • Requires transportation to an alternate address - use the Short Term Rider form.

The personal information collected on this form is to manage and provide yellow busing service and is authorized under the provisions of the Education Act and its regulations and also under Section 33(c) of the FOIP Act. All personal information collected during the process will be used by Transportation to ensure safety and manage student busing. If you have any questions or concerns regarding the collection or intended use of this information please contact Student Transportation.
