I hereby give permission/consent for Laffitte Academy to use my child’s photograph and likeness in all media publications, and on the Laffitte Academy website & social media. I release them from any expectation of confidentiality for the undersigned minor child and myself, and attest that I am the parent or legal guardian of the child listed below. 
Release and Waiver of Liability and Indemnity *
I agree that the student’s use of Laffitte Academy facilities, equipment, services or premises may involve risk of injury to persons, including those injuries described below. Student and Parent assume full responsibility. Parent and Student hereby release Laffitte Academy, it’s directors, employees and agents harmless from all liability for any loss or damage, for injury to student, whether caused by the active or passive negligence of Laffitte Academy. Student agrees to indemnify Laffitte Academy for any loss, liability, damage it may incur due to the presence of the Student in, upon or about the Laffitte Academy premises, facilities, services or equipment. Parent represents that (a.) the Student is in good physical condition and has no disability, illness, or other condition that could prevent Student from exercising without injury or impairment of health, and (b.) that Student has consulted a physician concerning an exercise program that will not risk injury to student or impairment of Student’s health. The risk of injury may include injuries arising in participation by Student in supervised or unsupervised activities at the school; injuries and medical disorders arising from exercising at the School such as sprains, broken bones, torn ligaments, torn muscles, heart attacks and strokes and accidental injuries occurring anywhere at the school. Parent agrees that the foregoing release, waiver and indemnity agreement is intended to be as broad as is permitted by the laws of the State of Florida and that if any portion is held invalid, it is agreed that the balance shall continue in full force and effect.
Policies and Guidelines *
Financial Policies

- Payments are due the 1st and should be completed by the 7th of the month, either in person during office hours, by mail, or online if applicable. Acceptable forms of payment are check, cash, credit/debit card, money order or PayPal. 
A $15 late fee will be required if tuition payments are not received between the 7th & the 15th of the month. If payments are not received until after the 15th of the month, a $30 late fee will be required.
A $25 charge will be assessed for each returned check and a $15 charge for each declined credit/debit card transaction.
Students whose accounts are deficient by 30 days will not be admitted to class until their account balance and all late fees are paid in full.
- Transactions are non-refundable.

Class Withdrawal and Re-Enrollment

- Withdrawal from a class or the school must be done so in writing and submitted to the school office either in person or by email. If a withdrawal is not submitted in writing, it is not valid and the account holder will be held financially liable to complete tuition payments until the withdrawal is formally received by the school office.

Absences and Class Make-Up Policy

- Please notify the school office of any absence as soon as possible, before the scheduled class time, either by phone or email.
- Make-up classes must be taken within a month of the missed class.
- Students may make-up missed classes in their current level or one level below. They may make up a missed class at a higher level, but only at the discretion and approval of their teacher.
We will not deduct a missed class from your monthly payment. If your child is absent
and is unable to take a make-up class, you are still required to pay the complete
tuition amount.
- It is important for students to maintain good attendance in their class as too many
absences may affect a student’s progress and future class placement.

Tardiness

- Parents are responsible for knowing class times and scheduled rehearsals.
- Students are not permitted to enter class more than 15 minutes late.
