Policies and Guidelines
Financial Policies

- We are a tuition-based school, meaning each student is responsible for paying a set amount by the end of the school year, based on the number of weekly classes they are enrolled in.
- Payments are due on the 1st and should be completed by the 7th of the month. Acceptable forms of payment include, cash, or credit/debit card. 
- ALL credit & debit card transactions will be assessed a transaction fee of 4%. Automatic Payments will be assessed a transaction fee of 4%.
- A $15 late fee will be required if tuition payments are not received within ten days of the scheduled due date. 
- Months are billed in full, regardless of absences, vacations or school breaks. Holidays are not prorated. Students whose accounts are deficient by 30 days will not be admitted to class until their account balance and all late fees are paid in full.


Class Withdrawal and Re-enrollment

- Withdrawal from a class or the school must be done so in writing and submitted to the school office. If a withdrawal is not submitted in writing, it is not valid and the account holder will be held financially liable to complete tuition payments until the withdrawal is formally received by the school office.


Absences and Class Make-Up Policy

- Please notify the school office of any absence as soon as possible, before the scheduled class time, either by phone (786-593-3002) or email (laffitteacademy@gmail.com)
- Students may make-up missed classes in their current level or one level below. They may make up a missed class at a higher level, but only at the discretion and approval of their teacher. Make-up classes must be taken within a month of the missed class. We will not deduct a missed class from your monthly payment. If your child is absent and is unable to take a make-up class, you are still required to pay the complete tuition amount.
- It is important for students to maintain good attendance in their class as too many absences may affect a student’s progress.
- We follow the Miami Dade County school calendar, so when county schools are closed, our school is also closed

Tardiness

- Parents are responsible for knowing class times and scheduled rehearsals. Students are not permitted to enter class more than 15 minutes late.

Uniform Policy

- All students must come to class in a clean, neat uniform. Hair must be neatly pulled back in a bun, if the student has short hair, the hair must be pulled back with clips away from the face.

Release and Waiver of Liability and Indemnity
I acknowledge and agree that the student’s participation in classes, rehearsals, performances, competitions, field trips, private lessons, and any other activities offered by Laffitte Academy involves inherent risks of physical injury. I voluntarily assume full responsibility for any and all risks of injury, illness, disability, death, property damage, or financial loss that may occur, whether arising from supervised or unsupervised activities, and whether occurring on or off Laffitte Academy premises.

I hereby release, waive, discharge, and hold harmless Laffitte Academy, its owners, directors, instructors, employees, agents, volunteers, and affiliates from any and all claims, demands, actions, or causes of action for loss, damage, injury, or illness sustained by the student/participant, whether caused by the ordinary negligence of Laffitte Academy or otherwise. I further agree to indemnify and hold harmless Laffitte Academy from any loss, liability, damage, or cost incurred as a result of the student’s participation or presence in any Laffitte Academy activity, facility, service, or use of equipment.

I represent that the student/participant is in good physical and mental condition and does not suffer from any medical condition, illness, or disability that would prevent safe participation. I acknowledge that Laffitte Academy assumes no responsibility for injuries or illnesses resulting from the student’s physical condition or athletic activities, including but not limited to sprains, broken bones, torn ligaments or muscles, heart attacks, strokes, or accidental injuries occurring anywhere during participation in Laffitte Academy activities.

In the event of injury or medical emergency, I authorize Laffitte Academy to obtain emergency medical treatment deemed necessary. I understand and agree that I am solely responsible for all medical expenses, transportation, and related costs, and I release Laffitte Academy from any claims related to such medical treatment or services.

I expressly consent to Laffitte Academy instructors gently placing their hands on the student solely for the purpose of proper dance instruction and correction. This agreement is intended to be as broad and inclusive as permitted by the laws of the State of Florida, and if any portion is held invalid, the remaining provisions shall continue in full force and effect.

By signing below, I acknowledge that I have read, understand, and voluntarily agree to all terms of this waiver.

Media Release

I hereby give permission/consent for Laffitte Academy to use my child’s photograph, videos and likeness in all media publications, and on the Miami Youth Ballet website. I release them from any expectation of confidentiality for the undersigned minor child and myself, and attest that I am the parent or legal guardian of the child listed below.
