Required COVID-19 Waiver (The “Agreement”)

By signing, you are agreeing to follow our social distancing and safety protocols.

I (the undersigned) acknowledge, Zen Fitness and Wellness, LLC and 7 Vines Vineyard have taken strict measures to prevent the spread of COVID-19 (such as social distancing, staggered classes, deep cleaning, etc.). I acknowledge that attending classes at Zen Fitness and Wellness, LLC and 7 Vines Vineyard could result in COVID-19 infection. I understand that Zen Fitness and Wellness, LLC and 7 Vines Vineyard, their members, and their staff are not responsible for any COVID-19 related illnesses or expenses. I hereby by my knowledge of this day am in good health and do not have COVID-19 nor show any symptoms. I understand that if tested positive for COVID-19 after attending a class at Zen Fitness and Wellness, LLC or at 7 Vines Vineyard, that I need to inform Zen Fitness and Wellness, LLC as soon as possible.

I understand that the activities that I will participate in  may cause serious or minor injuries, including bodily injury, damage to personal property and/or death. On behalf of myself, I waive all claims for damages, injuries, illnesses, and death sustained to me or my property. By signing this waiver, I understand and take full responsibility of any risk of attending and participating in classes at Zen Fitness and Wellness, LLC and at 7 Vines Vineyard.

By this waiver, I understand any risk, take full responsibility and waive any claims of personally injury, COVID-19 infection, death, or damage to personal property associated with Zen Fitness and Wellness, LLC and 7 Vines including, but not limited to receiving yoga training at the facility or at an outdoor or private event, using the facility and its equipment in any manner and participating and engaging in yoga, HIIT, Boot camp, Cardio Drumming, and any other classes led by Zen Fitness and Wellness, LLC- on or off the premises.

I have read, understood, and fully agree to the terms of this Agreement. I understand and confirm that by signing this agreement I have given up considerable future legal rights. I have signed this agreement freely, voluntarily, under no duress or threat of duress, without inducement, promise or guarantee being communicated to me. My signature is proof of my intention to execute a complete and unconditional WAIVER and RELEASE of all liability to the full extent of the law. I am 18 years of age or older and mentally competent to understand this waiver.
