ALL FACIAL SERVICES:

I have not been exposed to excessive sun and my skin does not feel sensitive or irritated in any way.

I have not had any facial waxing within seven days of receiving a chemical peel or 48 hours before the other facial treatments. If I have, I will inform my esthetician, so that she may take the appropriate steps to protect my brown area.

Prior to receiving treatment, I have been candid in revealing any condition that may have bearing on this procedure, such as: pregnancy (if so, consult your physician prior to treatment), recent facial surgery, botox/fillers, allergies, tendency to cold sores/fever blisters, or use of topical and/or oral prescription medications such as tretinoin, Retinol-A, isotretinoin, Accutane, Differin, Tazorac, Avage, EpiDuo or Ziana.

I understand that to achieve maximum results, I may need several treatments.

I understand there may be some degree of discomfort such as stinging, pin-prickling sensation, heat or tightness.

I understand there are no guarantees as to the results of this treatment, due to many variables, such as: age, condition of skin, sun damage, smoking, climate, etc.

I understand this treatment is a cosmetic treatment and that no medical claims are expressed or implied.

I understand that although complications are very rare, sometimes they may occur and that prompt treatment is necessary. In the event of any complications, I will immediately contact the clinician who performed the treatment.

I understand that I should follow my clinician's recommendations for post-procedure skin care to minimize side effects and maximize results.

I understand that extended direct sun exposure may be prohibited 72 hours post treatment, and the daily use of sunscreen protection with a minimum SPF of 30 is mandatory.

I understand that I must wait one week post-treatment to get facial waxing or threading.

ADDITIONAL INFORMATION FOR CHEMICAL PEELS

I have not had any other chemical peel of any kind within 14 days of this treatment. I understand I cannot have another chemical peel within 14 days of this treatment, whether it is performed at this location or any other location.

I understand I may or may not actually peel and that each case is individual. I understand that the amount of peeling does not correlate with degree of improvement.

I agree to refrain from tanning in beds or outdoors while I am undergoing treatment, and during the 14 days prior to and following the end of treatment. This practice should be discontinued due to the increased risk of skin cancer and signs of aging.

I understand I may need to avoid heat sources, including hot showers, saunas, and cardio workouts that may produce sweat for 24 hours, and 48 hours for chemical peels.

I understand that congestion and trapped sebum puss may be released, resulting in a superficial breakout.

I understand that - Underlying pigment due to the aging process, may move to the surface.

ADDITIONAL INFORMATION FOR DERMAPLANE:

I understand I may need to avoid heat sources, including hot showers, saunas and cardio workouts that may produce sweat for 24 hours, and 48 Horus for chemical peels.

I understand that micro cuts may occur during a dermaplane treatment

LASH LIFTS CONSENT

I understand that there are risks associated with Lash Lift and Tint Procedures

I understand that this procedure requires my lashes to be glued on the pad with a water based adhesive and lifted with an advanced solution, conditioning cream, and nourishing oil

I understand that as a part of the procedure eye irritation, pain, itching and discomfort may occur

I understand and agree to follow the aftercare instructions provided by my technician

I understand that I must not wet my lashes 48 hours post treatment

I understand that failure to follow the aftercare instructions may cause an undesirable result

I understand that my eyes needs to be closed for up to 75 minutes during the procedure and that opening my eyes at any point may cause the product to enter my eye and cause redness
