Patient & Family Advisory Council Member Application
The experience of care as perceived by the patient and their family is an important factor in healthcare, particularly as it relates to safety and service. As part of our ongoing efforts to provide high quality of care, we are interested in the perspectives of our patients and their families.

The Shirley Ryan AbilityLab has a Patient & Family Advisory Council. This Council is designed to foster the input from patients and their families regarding programs and policies, as well as identify opportunities and partner with staff to promote patient family-centered care.

Meetings will be held one evening per month for no longer than two hours. If you are interested, you do not need experience working in health care to apply. Our goal is to learn about your particular experience with healthcare at the Shirley Ryan AbilityLab.

Please complete this short questionnaire. If selected as a member of the Council, members commit to serve on the Council for one year and attend at least 75 percent of meetings. Select applicants will be contacted to schedule an interview. Again, we greatly appreciate your interest. If you are not contacted to serve, please know that this is solely based upon the needs, your focus (e.g., diagnosis), open spaces for membership, and composition of the Council at the time.
