(6x9"?) as well as how many pages it will be and the type of paper if you're using Amazon (black and white, or cream). Those options are needed in order to get a blank template for their printing service. It's okay if you don't know the page count yet.

Will you need the merchandising rights?
This is the art without typography in a high resolution so you can use it to sell prints/posters/other merch. Not every author will need this. Print rights to use the art for your cover are included already, this is separate from that. (this adds $250 to the total).
