Does your young person have any medical condition/allergy or take any medication?

In the unlikely event of an emergency, it is important that the person in charge of the group has necessary information about any medical condition/allergy that could affect your young person's treatment, and to be able to contact a parent/guardian or other member of your family/friend.  Although we try to ensure that a first aider is available, should any serious injury/illness arise we may need to call the emergency services and therefore need your consent to any emergency medical treatment which may be necessary.  Whenever medical advice or treatment is needed, the assistance of a GP or A&E department will be sought.  The Children’s Act 1989 allows a doctor to provide any necessary treatment for the purpose of safeguarding and promoting the child’s welfare.  I consent to any emergency/surgical treatment which my son/daughter may require during the time with us.  I understand every effort will be made to contact me.

The organisers may take photographs and videos at events. This is for the purpose of keeping memories, to share with the young people and online (Rooted webpage, Facebook Page and Instagram) and for future publicity purposes. To comply with the Data Protection Act 1998, permission must be granted by the parent or guardian before images of your young person are taken or used. Are you willing for us to include your young person in photos/videos?

The organisers use email to communicate about events. I consent to being contacted in this way.

Procedures followed are in line with the organiser's safeguarding policy. All information requested will be treated in confidence. It is in your interests that full and accurate information is given. We would very much like to keep in touch with you. By completing this form you are confirming that you are consenting to the organisers holding and processing your personal data in compliance with current UK Data Protection law. We would also like to keep in touch with you to inform you about Rooted news and events.

The leaders will try to eliminate all risks involved in any activities and designated members of the organising team will oversee the group during all event times. I understand that while the leaders are in charge and taking all responsible care, they cannot necessarily be held responsible for any loss, damage or injury suffered by a young person at an event. I consent to my young person attending Rooted events and taking part in the activities of the group.
