Thanks so much for your interest in training with me! Before we proceed, Iā€™d like to get some more details from you regarding your history, goals, and needs. I also want you to feel comfortable asking me any questions you might have. I want to make sure that Iā€™m the best fit for what you need, so I want communication to run openly both ways. Please find below a short questionnaire. Once submitted, we'll review your answers and respond within the next day or two. šŸ˜Š

Are you a member of a gym now? If not, are you willing to join one? (If not, do you have any exercise equipment at home?) *
Are you willing to keep a journal of your nutrition, workouts, sleep and moods and submit that to me weekly or monthly? *
