Online Beans & Strings Lesson Registration

RESCHEDULING & CANCELLATIONS
Your lesson will be scheduled as per your completion of the form below. This will be your lesson time every week (regardless of holidays or school system breaks) unless prior arrangements are made otherwise. In the event of cancellation with 24-hour notice, reasonable effort will be made to reschedule. The lesson must be rescheduled to occur within two weeks of the missed lesson according to the instructor’s available time. If this is not possible, the cancellation will simply be a missed lesson. Exceptions to the cancellation policy are not given for any reason – tuition pays for the reservation of a weekly lesson slot, not just the actual instruction that happens during that time. If a month includes a fifth lesson, the extra lesson is taught with no extra charge and is “banked” for use in case of cancellations by the student or on account of holidays. Beans & Strings will make note of banked lessons.

PAYMENT & REFUNDS
Tuition payment is due at the first lesson or by the 10th of the month (whichever is earliest) and is non-refundable. Tuition not paid by the 10th accrues a $10 late fee; please include late fee when remitting late payment. Lessons that are cancelled and cannot be rescheduled will not be deducted from the following month’s tuition. Returned checks will be charged a $30 NSF fee. Tuition may be paid by check, cash credit card. We offer a direct draft option. Please ask for an authorization form in-store. Direct draft accounts receive a $5 discount per invoice per month.

REGISTRATION AGREEMENT AND LIABILITY RELEASE
By signing this form you agree to policies set by Beans & Strings, LLC and release Beans & Strings, LLC and its staff in the unlikely event of an injury sustained by yourself or your student(s) during the course of or as a result of this musical activity. You also agree to the use of the likeness of the student on the Beans & Strings, LLC website or in the store.
