Is there anything that affects your ability to paddle safely at all times, or that we should know in an emergency? It might not preclude you from paddling, but it should be declared here. If in doubt, please consult your doctor.

Which type of membership? *
Canoeing is undertaken entirely at members own risk. Members agree to abide by the Club Constitution and Rules. Canoeing and Kayaking are “Assumed risk, water contact” sports, that may carry attendant risks. Participants should be aware of and accept these risks, and be responsible for their own actions and involvement. I agree that neither the Club nor its committee or officers can be held responsible for any accident or damage arising at the Clubhouse or while canoeing. If Hampton Canoe Club Ltd is ever wound up I will contribute up to one pound sterling to help meet its debts. I have explained the following membership terms and conditions to other members include in this application and have their agreement to these conditions also. *
