Please use this form to apply for Aftercare for the 2025-2026 School Year. This is first-come, first-served. This is not a guarantee that you will have a spot in Aftercare.
In addition to filling out this form virtually, it requires a $25 deposit per family. Only one deposit is required per family, regardless of the number of children registered. Again, this does not guarantee a spot for your child(ren) in the Aftercare program. This application is not valid until the $25 deposit has been received in the office.
Registration Payment may be made by check payable to SMOS. Please turn it into the school office. Attention: Christine Stephens.

Aftercare Pricing: The cost for aftercare for the 2025-2026 school year will be a $195 per month for one child and a $175 for each additional child. The partial months of August and May will be charged $50 per week for one child and $45 per week for each additional child.
