New clients for Megan's Pet Sitting must complete this form to the full extent prior to any pet sitting (only need to fill out once!) 
Please note, the photo's posted on social medias will be sent to you prior to being posted. I do have a professional camera that I will take pictures of your dogs with, BUT if your dog is too nervous around the camera or if I have any other reason not to have it, photos will be taken with an iPhone.

IMPORTANT NOTE
I reserve the right to decline any client or pet. A contract will need to be signed prior to any pet sitting care. Please fill this form out completely. 
If any emergency care is needed the pet will be taken to the primary veterinarian during their open hours, otherwise the pet will be taken to Allied Emergency Vet Clinic at the cost of the owner. I hold full discretion on this matter.
Cancelation is at no extra cost as long as it is at least a week prior to stay, under 6 days notice will be subject to a $20 cancellation fee!

Charging Notes
I charge by the afternoon, if you are home before 12 PM you will not have to pay for that day.
For example, if you are home after 12 PM you will be charged for that day. 
You will be charged for the first day regardless of time unless discussed with Megan prior to stay.
