Questionnaire for parents who have children with type one diabetes
Hi, my name is Jasmine and I'm a 4th-year industrial design student at OCAD University, I'm doing a thesis project related to children with diabetes, I would like to ask some questions to parents /caregivers as my subject matter experts pertaining to food-related issues. Thanks for helping me in my thesis research. Below are some questions that I have, you can leave blank if you do not want to answer some of the questions. How old is your child?
How long has your child been diagnosed with T1D?
Is your child aware of his/her diabetes diagnosis? If yes, how does he or she feel about the changes in their everyday life? (acceptance, apprehensive/fear, cooperative?)

How many data does your child need to face every day
When your child been diagnosed, does your child knows the importance of nutrition management? What kind of behaviours does she/he have?
As parents, what are the most challenging part with complex care and food management?

What kind of behaviours and emotions appears when dealing with diabetes care? (child's behaviours and emotions) (If it is ok, could you give an example?)
How many devices need to be used every day? How does your child think about these devices and do they have the stress when looking at the numbers?
Are there any difficulties when the child is at school?
When you start to manage the food plan for your child, do you find it is easy or complex? Why? List some examples of challenges you face when making food management if any?
How you usually communicate with/educate your child about how to take care of diabetes (ex: what food will benefit their level of blood sugar stay in normal, etc.)?
How is your child's motivation for doing different tasks? (Injection Insulin, Following the meal plan, Nutrition counting and learning, etc.)
How much of the diabetes care do you provide for your child?
How is your child's understanding of nutrition knowledge?
Do you think it is important to let children learn nutrition management? Why?
