All-Inclusive Release of Liability Waiver
Chicagoland Fat Loss Camps (CFLC)

I understand that I am voluntarily choosing to participate in a workout class at Chicagoland Fat Loss Camps (CFLC). I know that exercise involves some risk of injury, including but not limited to: pulled or strained muscles, sprains, slips, falls, broken bones, heart problems, or other serious injuries. By signing this form, I agree to take full responsibility for any injuries I may get during or after participating in the class.

I understand and accept all risks involved and agree not to hold the following people or organizations responsible for any injury, illness, or loss:

● Chicagoland Fat Loss Camps (CFLC)

● New Progressive Missionary Baptist Church (Lessor)

● Matteson Community Center (Lessor)

● Travis Garza Fat Loss Camp

● TLC Fitness

● All trainers, instructors, staff, consultants, owners, and anyone else involved with CFLC

I know that these physical activities come with natural risks, and I understand that injuries could happen due to:

● Overexertion

● Improper use of equipment

● Slips, trips, or falls in the rented space where classes occur

● Unknown health problems

● Negligence of an instructor or another participant

I also understand that the spread of COVID-19 and its variants is out of anyone's control. Because it's impossible to know exactly where or how the virus was caught, I agree not to hold any of the people or groups listed above responsible for any illness, related medical bills, loss of income, or even death.

In addition, I understand that none of the people or organizations listed above will be responsible for any stolen items, property damage, or personal loss that may happen during my time at the class.

I also agree to the following:

● Notify the Instructor:
I will let the instructor know right away if I have any medical conditions or limitations that may affect how I exercise.

● Follow Instructions:
I will follow all safety instructions and exercise at a level that is safe for me.

● Release of Liability:
I fully release and discharge CFLC, its staff, instructors, owners, and affiliates from any and all claims, demands, or causes of action related to any injuries I may suffer, even if those injuries are caused by someone’s carelessness or negligence.

By checking below, I confirm that I have read, understood, and agree to the terms of this waiver. I understand that I am giving up certain legal rights.
