Prayer Counseling Intake Form
Thank you for inquiring about Prayer Counseling Ministry at Calvary Church. The intake is confidential and will only be seen by the Prayer Counseling Pastor, Desiree Elrod. At this time the Prayer Counseling Ministry office is closed until Monday, September 7th.  If you want to be apart of the list of individuals who are seeking to be scheduled for the fall, please fill out the intake. I can't guarantee a spot will be available for you until January. However,  I will know more when I begin the process of scheduling at the end of August. Either way, I will keep you informed. 

If you have not been seen before I will begin with a consultation so I can do an intake and explain what PCA is. After attending a consultation, 5 prayer sessions will be offered if you're wanting to move forward.

This is a ministry that several churches utilize. Individuals are trained on coming alongside others who are processing loss and are dealing with heavy burdens. . This is offered for free and the individual will meet as long as you desire.
