This form is to be used by fundraising teams for Remember Betty interested in hosting an in-person or virtual event for the foundation.
All events need to be approved by Bethany and Danny.

Who will be the point of contact for the event? Please include full name, email address, and phone number for your team captain or team member in charge.
Will this be a virtual or in-person event?
What is your preferred date and time for the event? Please include time zone. Also, please include an alternate date and time that could work for the event.
What date would you like to begin promoting your event and need graphics and social media assets ready by?
Please describe your event in detail. If virtual, what platform will you be using for the event? If in-person, where will the event be held? How will people register for the event? Are any third-party vendors or companies involved? What is the theme of the event? Is there a schedule, etc.
Do you need a signed item for your event?
