This UPMin Health Check Scorecard is used for monitoring UP Mindanao Constituents and Visitors' health condition upon entry to the campus. This form has a scoring system that will provide the user the appropriate action to be taken in relation to their health conditions before entering the premises of UP Mindanao.
To determine the decision category and recommendation based on total score, please refer to Score Equivalent Section of this Scorecard.
0 = Decision I
He/she is required to wear mask indoors and observe social distancing.
1-2 = Decision II
Personnel/Student/Visitor is allowed to enter but he/she is required to wear mask at all times and observe social distancing.
3-5 = Decision III
Personnel/Student/Visitor is NOT ALLOWED to enter UP Mindanao. He/she shall coordinate with their Direct Supervisor or Instructor for proper instruction on his/her work/ studies arrangement.
Guest will be denied entry in any offices.
6-10 = Decision IV
Personnel/Student/Visitor is PROHIBITED entry in the office and advised to see a Medical Doctor.
